The Three Instruments of Peace is designed to help you plan ahead with peace of mind. It includes three important legal and medical services: Will, Enduring Power of Attorney, and Advance Medical Directive.
Evangel Hospital now offers the Three Instruments of Peace service. Through one-on-one assessments conducted by our registered doctors, we ensure your wishes are documented clearly, legally, and will be respected in the future. This is especially important for everyone facing later life or unforeseen circumstances.
Witness Requirements for the Three Instruments of Peace:
Advance Medical Directive (AMD)
- Registered Doctor: Must ensure that the person making the Advance Medical Directive possesses the mental capacity and fully understands both the legal validity and medical implications of the directive.
- Second Witness: Must be at least 18 years old and must be a disinterested party (e.g., not a beneficiary of the person’s will or insurance policy).
- If the person is unable to arrange a second witness, Evangel Hospital can appoint a qualified healthcare professional from our team to serve as the second witness in accordance with legal requirements.
Enduring Power of Attorney (EPA)
- Registered Doctor: Certifies and is satisfied that the donor has the mental capacity to execute the document.
- Lawyer: Certifies that the donor appears to have the mental capacity to execute the document.

The registered doctor and lawyer must not be the appointed attorney, the attorney’s spouse, or anyone related by blood or marriage to the donor or the attorney.
Advance Medical Directive (AMD)
Advance Medical Directive (AMD) — Mental Capacity Assessment
Our doctor will explain in detail the practical implications of various life-sustaining treatments (including cardiopulmonary resuscitation, artificial nutrition, etc.), helping the individual express their personal wishes regarding end-of-life care while they are still mentally capable.
The doctor will conduct a one-on-one mental capacity assessment to ensure the person has the requisite mental capacity, possesses full decision-making capabilities, and clearly understands the nature and consequences of their instructions.
According to the law, two witnesses are required for an Advance Medical Directive, one of whom must be a registered medical practitioner. Our doctor will serve as one of the witnesses and sign the relevant documents.

Enduring Power of Attorney (EPA)
Enduring Power of Attorney (EPA) — Mental Capacity Assessment
Our doctor will conduct a one-on-one mental capacity assessment for the donor to ensure they clearly understand the meaning of the Enduring Power of Attorney, the scope of authority, and the roles and responsibilities of the appointed attorney(s).
The doctor will also confirm that the donor is not signing under undue influence or while mentally incapacitated, and will issue a medical certificate. The doctor will act as one of the statutory witnesses and sign the document on the day of execution.

(*Excludes outpatient consultation fees. The testator must independently arrange for a lawyer to draft the relevant legal documents.)

Last Will and Testament
Will — Mental Capacity Assessment
Our doctor will conduct a one-on-one mental capacity assessment for the testator to ensure they fully understand the legal implications of the will, the scope of their assets, and the arrangement of beneficiaries.
Upon completion of the assessment, the doctor will issue a medical certificate to strengthen the legal validity of the will and significantly reduce the risk of future disputes over inheritance.
